Purpose: Use this form to request records from the Department of Motor Vehicles (DMV) in accordance with the Virginia Freedom of Information Act (FOIA) (VA. Code § 2.2-3700 et seq). Please note, the release of personal, driver, and vehicle information from DMV records is governed by VA. Code § 46.2-208 through 46.2-214 and the Federal Driver's Privacy Protection Act (18 USC §§ 2721 through 2725 ). You may only receive such information if your request meets one of the release conditions under Virginia law.

Instructions: Provide as much information as possible. Not all fields are required; however, DMV must be able to identify and locate the records you request. If requesting driver, vehicle, or personal records, including crash reports, please complete and upload an Information Request form ( CRD93 ). If requesting information for Civil Immigration Enforcement purposes, you must upload supporting documents, as required by Virginia law. You may use this form to submit consent from the subject ( CRD93 recommended) or a lawful judicial order. Judicial subpoenas or judicial warrants must be served or executed in accordance with applicable state or federal law.

Note: Your request must identify the records you are seeking with reasonable specificity, meaning it must be detailed enough for us to identify and locate the requested records. Your request must ask for existing records or documents. FOIA gives you a right to inspect or copy records. FOIA does not apply to a situation where you are asking general questions about the work of DMV, nor does it require DMV to create a record that does not exist. DMV must notify all requestors that a “public body may make reasonable charges not to exceed its actual cost incurred in accessing, duplicating, supplying, or searching for requested records”. You may request a cost estimate in advance of supplying the records. If a cost estimate is not requested, DMV will proceed to process your request and notify you of any fees, if applicable, with our response.
